STANDARD PHOTO RELEASE STATEMENT

I hereby authorize the City of Pembroke Pines/City Connect to publish the photographs taken of me, and my name, for use in the City's printed publications and website. I acknowledge that since my participation in publications and websites produced by the City of Pembroke Pines is voluntary, I will receive no financial compensation.

I further agree that my participation in any publication and website produced by the City of Pembroke Pines confers upon me no rights of ownership whatsoever.

I release the City, its contractors and its employees from liability for any claims by me or any third party in connection with my participation.

STANDARD PHOTO RELEASE STATEMENT FOR MINOR CHILDREN

I hereby authorize the City of Pembroke Pines to publish the photographs taken of me and/or the undersigned minor children, and our names, for use in the City Connect printed publications and website.

I release the City of Pembroke Pines from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorize the City of Pembroke Pines to use their photographs and names.

I acknowledge that since participation in publications and websites produced by the City of Pembroke Pines is voluntary, neither the minor children nor I will receive financial compensation. I further agree that participation in any publication and website produced by the City of Pembroke Pines confers no rights of ownership whatsoever.

I release the City of Pembroke Pines, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.
